As a Care Manager, you will deliver high-quality, comprehensive care to customers within the Home Care Packages Programme. You will utilise your specialised clinical expertise to ensure effective support at home, and develop individualised care plans that address complex needs. You will manage daily administrative tasks, support program operations, and provide regular reports to the Program Manager on key performance indicators (KPIs) and budgetary performance.
